Program Description

Event Details

A note from the Healthy Communities AmeriCorps Program at Community Health Center, Inc.   Unfortunately, our program has been impacted by the recent funding cuts which has caused our program to be currently suspended. At this time, our AmeriCorps members are not in service and will not be able to conduct this session that was scheduled for May 14th. I do apologize for any inconvenience that this has caused.  In the event that our program is reinstated, I will be sure to reach back out.

As AmeriCorps members with the Healthy Communities Program at Community Health Center (CHC), our overall goal is to increase health literacy and healthcare access for those in our communities.

Are you interested in learning about:

  • Free or Low-Cost Health Care
  • Tips for Talking With Your Medical Provider
  • Pharmacist Healthy Food Options
  • Patient Rights and Responsibilities
  • Chronic Diseases
  • Services Available at CHC

Join us at this one hour healthcare education session presented by AmeriCorps, Community Health Center (CHC), and West Hartford Bloomfield Health District (WHBHD). To help our partners continue their program, you will be asked to complete a short questionnaire at the beginning and end of the program.

This program is in-person at McMahon Wintonbury Library, 1015 Blue Hills Ave. Limited space available.
